NDC Charges in PPM 6.5

A power tracker table will need to be setup named NDC.

There are 5 different elements to be completed for the NDC requirements for each line item.� In patient registration click the power tracker button and choose the hemocrit template.

1. NDC Qualifier will always be N4

2. NDC Number should be entered in line

3. Unit of Measurement which is either ML GR UN or F2

4. Measurement amount

5. Prescription number, if applicable

Example: If the fourth charge is a code that requires the NDC number, only fill in the line that begins with LN4.

Hematocrit Charges in PPM 6.5

There are 5 different elements to be completed for the Hematocrit requirements for each line item.� In patient registration click the powertracker button and choose the hemocrit template.

1. The test results will be TR

2. Enter R1 for Hemoglobin results or R2 for Hematocrit results

3. Numeric test results

4. Hematocrit Test Date�� Enter the date in YYYYMMDD format.

����� Example: 5/17/2008 would be 20080517

5. Creatine Test Date�� Enter the date in YYYYMMDD format.

Example: If the fourth charge is a code that requires the Hematocrit level, only fill in the line that begins with LN4.
